What are your water params (NO3, salinity, temp, pH, Alk, Ca)? What nutrients are you adding to the water (foods how much and how often)? Are you using RO/DI water for top-off? Did you start with RO/DI? How long has the tank been up?

What sort of janitorial crew do you keep? (snails, hermits, etc...)

Without knowing more about your system its kinda hard to tell what the problem is. Two things I would check first though, Check the TDS of your water supply. Are you using RO/DI? How long since you replaced you filter cartridges and RO membrane? Second, how old are your light bulbs? Are you running MH? Bulbs more than 6 months old?? Those are the first things I would check.
