Losing My Head(s)

Can someone give me an ideal why my branching hammer, frogspawn & candy will be looking great then one day one of the heads will look like they're melting then a couple of days later they just let loose from their skeleton and float off and die?

I've had them for 1-2 yrs, been in the same place, about midway up, gentle current. Mostly have leathers, mushrooms, gorgoinia, and a couple LPS: bubble, hammer (had), frogspawn x2, candy, fox. I have 2-175w MH, 2-55w 10000k PC, 2-55w 03 PC lighting on my 72-g bowfront. Ca=420, Alk=3, PO4=0.1 (was higher), NO2=0.1, NO3=5
