Bulb burned out to quick?

I have a 2x250 watt MH fixture from you guys. I've had it for about 3 months now, and one of the Coralvue bulbs I got from you has burned out. Isn't that a bit soon to loose a bulb (this is my 1st MH fixture)? Could there be a problem with the ballast causing the bulb to die?

BTW, I switched the bulbs and the dead one still did not fire.
