With such a yellow bar on the head, either A. dussumieri or xanthopterus.
I'd say A. xanthopterus.

Looks to me like an Eye-Stripe Tang, also known as the Dussimieri Tang. Very beautiful fish, and get large, too. Around 16-18"

Let me correct myself, Acanthurus blochii, Blochii Tang. Also attains the same size.
