lumpy forehead on angel fish HELP!

My blue ring angel has developed a lumpy, warty area all over his forehead. two weeks later the area developed white spots (secondary infection?). salinity .018, ammonia 0, ph perfect. Fish eats formula 1, mysis shrimp soaked in Zoe and garlic, green algae sheets. I have only had this fish for two months with one month being in a QT.
