Looks like that is right where an eye was. Almost looks like the flesh was bitten out. Watch to make sure you dont have something in the tank that has a taste for chalice. ;)

Should I cut it out (Frag ) or will it heal ?

Looks like it will heal. Just watch it and make sure it doesn't start getting worse. If it does, it may be time to frag the coral.

my buddy had a copperband that ate a few eyes out of his chalice colony just like that. His has healed just fine, you cant even tell. I would leave it for sure, should heal fine.

The only fish thatmay / may not be reef safe is a half black angel,
How would I get him out ?
125 gallon, 200# lr etc

Have a piece like that missing from 2 of my favias. Would/could a purple tang be munching on them? Holes look suspiciously like the shape of his pointy little mouth, but not convinced. As for your chalice, should heal fine unless something keeps eating it.

I have a yellow and powder brown tang, never seen them bite at it though.
I did see the halfblack nipping at where the hole is.
Going to get him out somehow

I used one of those fish trap things once and caught two pesty fish with it. I just put a little brine shrimp in the box and waited... I guess you gotta be pretty patient to use one of those thing though :)
