What can cause polyp/s in LPS corals to detach themself from the base/skeleton?

I don't know about LPS. But have heard with Zoas that's how they spread out to other places. So I've been told, and in my tank I agree. I have polyps popping up all by themselves in some areas.

is anything picking at the coral?

Nope,nothing.
I have to say,all my other LPS are fine and in excellent shape.
This is the only coral with that problem.
The polyps in the edges is actually shrink a little bit,peel off and detach,until there is nothing left.

move it! it doesnt like the spot you put it in. what kind of LPS is it? tank size? lights? params? and flow?

I'll try move it to another place.
It's a Favia.
75 gallon,two 250 watt MH + two 54 watt actinics.
flow - 3 pumps of 1250 GPH each one + 1 double pump of 3000 GPH.
the pumps are Sunsun (similar to koralia).
water parameters:
salinity - 1.025
Calcium - 450
Alk - 9
Mag - 1300
