As you know, many old powerheads and pumps that use the enclosed and submergable head, and propellor as the only moving part, don't always start on their own after some running time. Often, after a couple of years of running time, you may have to open the case to touch the propellor to get it to start.

Perhaps this is not because the propellor has stopped at the wrong place, degree of angle to start the spin. It is designed to stop at the degree in the spining circle so that the spindle will rotate without the propeller moving. may be for 330-340 degrees of the circle.

What do you think causes this problem?

Are most of these situations fixed if you just replace the spindle/propellor assembly?
