reef crashes help others avoid them

who here has had a reef crash and how did it happen? good topic to help otheres avoid it

While I haven't had one crash on me, I can say that staying on top of water chemistry will surely help prevent it.

From what I have read, changes even for the better can prompt a crash. Everything that is good for the tank should be done slowly such as lighting changes, water chemistry, livestock additions, etc
