maroon clown

My maroon clown was bloated one night all of a sudden the next morning he was dead... can anyone please tell me what happened and if there is a way to avoid this in the future??

Water parameters would be the best place to start.

salinity is at 1.026, temp i believe is at 76 F, I have a metal halide light, protein skimmer and filter all pre owned items so not sure exact make and models. also in the tank are a flame angel 6 line wrasse and a green chromis. couple snails a coral banded shrimp and a porcelain crab.
