How do you make a return pump run 100% of the time on the Apex?
Fallback: On
???

I personally would not run it on the apex. I would plug it straight into the wall.
If all else fails and there is still power I want my return pump working. If all of your apples are plugged into a controller and it dies so does the tank.

I do something like
Fallback ON
Set ON
IF Feed B then off (Feed B is my maintenance routine)

That's what the Fallback: On is for...
If the power goes out, it stays on and keeps running.
I just don't know how to make it stay on all the time as there are 3 different on/off/on commands in the default "Pump" selection.

Yep Set ON to make it run 100% of the time and Fallback ON to keep it on after the power goes out.
I only run mine off the Apex because I have a water on floor sensor that shuts off the return and skimmer.

Mine doesn't say SET?
It has 3x On/Off/On and times associated with each?

don't use the tool, just program the outlet manually.

It automatically has this when I select PUMP
Should I select a different type for the outlet?

Also, how do you know which EB8 is which on the display?
I have 2x EB8's connected and they show 8x outlets for each but I don't know which is which? Do I just have to do a test to figure it out or is it shown somewhere?

I'm always the minority in this discussion and even though my advice does not answer your question I would strongly consider using the advice instead of brushing it off.
I have seen the Apex's fallback circuit fail first hand. I have also seen fallback circuits on $40,000 controllers fail. I am not impressed by them at all.
Just a little caution... If you unplug the apex and the tank cannot survive then you have made your system weaker with the addition of a controller because it like all electronics WILL fail.
Anyways, I always :deadhorse: when it comes to return pumps and reef controllers so I will leave it at that.
