Clownfish dying?

I came home to my clownfish on the sand bed, breathing heavily. He will swim around but go right back to the sand bed after a while. I noticed his pec fins look like they are rotting a little and also there are some white spots on his tail... Could this be Ich? If so what should I do going about this?

He definitely has ich(from what I could see from the pic) and maybe fin-rot. Did you qt?
What are the other tankmates? How long has the tank been set up?

Tank has been set up for 4 years probably now.. There is just 1 other clownfish and snails and hermits. I was in the process of getting more fish but I guess that isn't happening anytime soon. I don't have a set up QT and I'm about to go on vacation for a week so I have no idea what to do.

And I'm not exactly sure how Ich works but on his fin it doesn't look like salt crystals like some say Ich looks like, it just looks like the pigment and color became transparent in those spots..
