Just noticed small Regal tang has 3 white spots and is rubbing onto the rocks.
What is the next step? do i buy any treatment?
Can i treat the fish in the main tank as it is a fish only.

ok can you treat the main tank with copper without removing the fish to a hospital tank
Only invert is a shrimp

No. You will kill all the beneficial bacteria in the rocks as well as your shrimp. ALL fish need to be treated in a proper hospital tank.

okay so i remove all the fish and not just the one thats infected to a hospital tank and treating with copper product like

Also read that even if you remove the infected fish and reintroduce the tank is stil infected so will get it again:sad2:

The display tank needs to remain fishless for 72 days, this will rid the tank of ich. All fish in the display must be removed and treated using one of the options the Tank transfer method, copper, chloroquine phosphate or hyposalinity.

okay read somewhere that its after 7 days the parasite goes to your live sand. Is this true maybe it hasent infected the whole tank yet?

no. it is copper.
The links posted above will answer a lot of your questions, as well as teach you other stuff about ich that you don't know to ask. If you are going to try to keep a tang in a 50 gal tank you will need to study up on all of this stuff.

Will i just leave the shrimp while it is sitting for this 72 days in the main tank and remove the rest of the fish

Yes, the shrimp and any other inverts such as snails or crabs stay in the display.
