What's wrong with my poor fish? He has a large, clear blister on his nose (the picture doesn't do it justice) and has trauma to it's tail. Is there anything I can do to help it?

If you don't have a hospital tank, the best you can do is not stress it more than needed. Feed it well and maybe make the night cycle a little longer as rest helps them.
If you do have a hospital system, you can put it there after a fresh water dip and treat it with ant-fungal and antibacterial. API Melafix marine is a good alternative. Look into prazipro as an alternative for in tank treatment although might not address exactly whatever your fish has. The tail appers to be some parasite related condition, so you may want to look into API's General Cure.
Finally, if you have a tankmate that's injuring him (which I doubt), address that issue as well.

There is a lot more going on with that fish than it's mouth.

oscarinw Thank you for the info! We do not have a hospital tank but I will be making the night cycle longer.
Dkuhlmann Aside from it's nose and injured fin, is there something alarming that you see?

Yes, the stomach, tail and possibly the spine. With the stomach like that I question if the spine is straight.

I am trying to research this fish as he's not looking any better. Can anyone tell me what kind it is?
