How did someone make a hybrid A. Achilles x A. Japonicus, a hybrid N. Lituratus x N. Unicornis, and a hybrid Z. Flavescens x Z. Scopas? I thought tangs have never been bred/raised from eggs in captivity before? How do they do it?

Tangs are group spawners and those hybrids occur quite regularly in the wild when mixed swarms spawn.
So far haven't heard anything about hybrids of hybrids it is possible that those accidental hybrids are sterile.

Right ..... wild caught not captive bred. Same for the dwarf angel hybrids.

So this occurs in the wild, that makes more sense. I was going to say, the larval stage of the genus Acanthuridae lasts a few months, so I'd presume it would be fairly difficult to maintain the larvae, being pelagic, as feeding them, and the possibility of getting sucked into pumps would be just a few challenges to overcome.

Some dwarf angels get already bred in captivity. And some of their hybrids were made intentionally.

Most are not.

Don't forget the Gem x Scopas and Black x Scopas :P

All wild fish...

As far as I know there are no tank raised tangs I've ever heard of.

Tank bred (meaning the parents spawned in captivity) and tank raised (fish were raised in captivity) are two distinctly different things.
The latter may have already been done from in the wild collected larva.

Attempts to breed tangs in captivity are currently done for example by Sustainable Aquatics: http://sustainableaquatics.com/reflections-on-sustainable-aquatics-moving-from-2014-into-2015-and-the-futur/

Tangs typically won't breed in captivity... Angels are a different story

Tangs can and do breed in captivity. The hard part is to raise the larval past a certain days. Read this: http://risingtideconservation.blogspot.ca/2014/02/yellow-tang-research-at-oceanic_24.html

They breed but out of 40,000 eggs only 150 were still alive at 50 days... More importantly no one wants to see a captive bred yellow tang, put that on the list with clownfish.
