just a guess but, internal parasite that could be causing a swim bladder issue

I have a Qt always set up. Any medication you suggest?

You can see in the pic her gills are flared out. Anyone know what causes that or how to treat it?

The male is normal so you can rule out water toxin as cause. Did you recently added anything into the tank? If not then the chance of infection is also unlikely.
I afraid it looks like your fish had a stroke. If infection is the cause, your fish would be dead by now. I think tank raise clowns often have internal defect and things like this happen more often. I don't think you can do anything. It will most likely up to the fish if she is going to do better.

Nothing has been added to tank in at least 3 months. Not even coral. I've had the pair for about 7 years. The male is perfectly normal still.
