Anyone know where to obtain a Bandit Angelfish?

Anyone own one successfully?

I was able to aquire some in the past out of hawaii (all of a decent size)- but it took me some crafty cherry picking and wheeling and dealing in LA to get them. They were not all that expensive, seemed to do well and actually ate while in my holding system. I didn't follow up on the people who took them, but there was an angel collector in the area who had one for at least 3 years.
That was about a year ago, and I have not seen anyone with them since, but keep your eyes open.

You may want to try and contact Reef Culture Technologies. They should be able to get one for you or put you in touch with someone who can. But they often don't do well in captivity. Their diet consists mostly sponge and converting them over to regular food can take some patience. They are also a deep water fish that often suffer from sickness related to decompression issues. Expect to pay for this fish as they are exclusive to Hawaii and aren't often seen in the hobby.
