UPDATE: Sorry I have been busy moving to my new house and didn't really had time to update this. The problem didn't go away, I lost almost all my corals including LPS. The tissue just keep separating from a skeleton. First it was just SPS, then all my torches, now hammerhead. I find tissue floating around the tank every day.

With a new house I build a new 150G tank and moved everything there. I used all new sand and new water. I only transfer the fish and rocks with whatever corals left. It has been a week since in a new tank but the problem is still persist. All parameters are in check. I used same salt to mix new water.
