Unhappy Disease Identification on Blue Tang

I am trying to determine what disease or infection my Blue Tang has in order to be able to treat it. I was thinking it might be bacterial, but now wondering if it might be Lymphocystis. The 2 small white spots on the Dorsal fin, and there is also one below the mouth. His one eye is also a little cloudy.

We have had him 2.5-3 weeks and the spots on the Dorsal showed up 4 days ago and started getting bigger. Originally we thought it was injury since he loves spending time in a crack in the rocks.

Any Help would be appreciated!
