PLEASE HELP Something is killing my animals

I don't know what's going on. A couple weeks ago my hammer literally melted. I have never seen anything like it. Then a few days latter some shrooms that was next to it melted. So I removed the whole rock from the tank. Now it seems that some hairy mushrooms on the other side of the tank are starting to melt away. Any advise would sure be a help.

Yup, gotta know that plus description of lighting and list of recent changes.
With your 8 main water parameters, a note of how long they have remained stable

Are you using kalkwasser in your ATO? (ie perhaps too much got released and had a pH spike)
Are your powerheads and return pump all functioning correctly (cleaned of algae to allow adequate flow)
Perhaps something died (like a snail) and polluted the tank.

Sadly, I almost completely lost my Hammer and to this day do not know what had happened. These are such fragile creatures that there are so many variables at play.
