That depends on what you are trying to accomplish: Deal with elevated phosphate, an algae problem, maintenance, demonstrated allelopathy, yellow water, etc.
I change mine every 2 weeks.

I'm going to be using it for preventive measures. As of now, my tank is 12 weeks
old and finished cycling about a week and half ago. I am just trying to go slow and think of warding off problems before the exist. |

I'd start off changing both at 2-4 weeks, and alter the time up or down (and amounts up or down) based on apparent need in the aquarium.
